Diabetes Self-Management Program (DSMP) workshops include 6 different session, each covering a new topic – meal planning, manage medications, fitness/exercise, healthy eating and blood sugar levels, develop problem solving skills, effective communication with health care professionals.

Dates (Wednesdays for 6 weeks): October 17, October 24, October 31, November 7, November 14, and November 21

Time: 1 pm to 3 pm

Location: CentraCare Clinic Albany – 320 3rd Ave Albany, MN 56307

Registration required by October 10, 2018 and a minimum of 10 and maximum of 15 is required for this workshop to be presented.

A suggested free will donation will be requested for this workshop. Cost of the participant book is $18. Snacks provided.
